To do so, complete the following form and click on the “Submit Customer Profile Information” button. Make sure you submit this form prior to emailing or faxing your first order to mailto:sales@laserband.com/(314)726-1028. Remember to include a copy of your W9 and a purchase order number. * Denotes required fieldsYour initial order must be placed via fax or email. All subsequent orders may be placed by the method that is most convenient for you: Fax, Phone or Email. We also accept orders via EDI using GHX (Global Healthcare Exchange). Please provide a purchase order number (PO#) on all orders. SHIPPING INFORMATION Freight is prepaid and billed to customer. Standard method of shipping is ground due to the case weight (Avg: 25-30# per case). Customer may request expedited shipping at their own expense. The following freight carriers are used most often: UPS and UPS Freight. Orders generally ship within 1-2 business days after receipt of order. All orders are confirmed via formal acknowledgement; please contact us if you do not receive an acknowledgement within 1 business day of submission. Requests to ship same day must be received by 1:00 PM CST. A credit limit and payment terms will be established at the time of your initial order. |
